Follow-up of Retention and VL Update and suppression
Description
Considdering a total of 40 clients who were absent during July and became IIT during this current month. It was urgent to work with the site team to ensure effective tracking and follow-up of these clients to ensure return to care.
Had one on one session with retention APS and did calls to ascertain responses in the tracking resgisters are updated
Also cases for home visits and community follow-up were identified for immediate tracking
Cases of death were analysed using files to verify effective follow-up was done and screening for AHD done
All other retention APS were engaged so as to track long abandones and ensure these clients are brought back to close the gap
