Description
Each APS was asked to bring out the files of their absences and LTFU to be tracked
-Amongst the 3 absences tracked, 1 came back today, 1 is taking treatment elsewhere and 1 was not reachable
-out the 51, 2 had come back.Out of the 18 clients tracked this day, 1 came back same day, 14 were not reachable, the contact persons of 2 clients will get to them and 1 was declared death since 2022 by the daughter following breast cancer.
Verification of T-outs
Amongst the 8 T-OUTs verified of the 16 T_outs in Q3, no health facility confirmed having received them. There is need to get back to these clients to find out where they are presently taking treatment.
Remark
APS ar not moved by the poor retention performance.
